Financing Orthodontic Treatment
Creating your great new smile often requires treatment over many months.
Drs. Chermak and Hanson provide several payment options to make the
treatment affordable for you. Payment can be made by check, cash, or
credit card. We accept all major credit cards.
Cost
The cost of treatment depends on the severity
of the patient's problem. You will be able to discuss fees
and payment options before treatment begins. We have payment
plans to suit different budgets, including a no-down payment
option. We also accept assignment from most insurance plans,
and file the necessary papers to the insurance company. We
work hard to make orthodontics affordable.
For your convenience, we offer three payment options:
Office Payment Plan - (no interest charges)- Ortho Banc
- No interest charges with automatic draft.
- Payment arrangements are listed on treatment summary
- Most major credit cards are accepted: Visa, MasterCard, Discover and
American Express
- A 10% family courtesy fee reduction is available for additional family
members.
Healthcare Finance (formerly Orthodontic Fee Plan) - (no down payment)
- No initial down payment required
- Take up to 36 months to pay with low monthly payments (includes a
finance charge).
- 48 month option available for multiple family members.
- Fast confidential service by phone (1-800-637-3393) for outside
financing through
Healthcare Finance.
- No family courtesy fee reduction is available when OFP financing option
is chosen.
Payment In Full (5% fee reduction)
- An administrative fee reduction of 5% is offered when a fee is paid in
full by check or cash at the start of treatment
- Fee reduction does not apply to payments made by credit cards or OFP
financing.
- Assistance is always available for submission of your insurance
claims.
- Benefits may be coordinated with your monthly payments and
thereby reduces your portion.
- Payments for orthodontic services are usually eligible for
"Flexible Spending Accounts".
- Only one fee reduction will be applicable to any individual
account.

Insurance Guidelines
We are happy to work with you and your insurance company to receive the
maximum benefit possible under your policy. We have found that the more
you know about your individual policy, the easier it is to process your
claim. To assist us both, we have provided the following answers to the
most commonly asked questions.
Please keep in mind that our primary financial relationship is with our
patients and their families and not with the insurance company. We
realize that understanding your orthodontic insurance can be difficult
at times and we welcome any questions you might have. Thank you in advance
for your cooperation.
How Does Orthodontic Insurance Work?
There are many different agreements between insurance carriers and their
subscribers, and each contract provides a different benefit. Orthodontic
insurance generally differs from regular medical insurance in that it rarely
pays for orthodontic treatment in full. The benefit is paid as a percentage
of the orthodontic fee until the maximum has been reached.
Each insured individual usually has a lifetime maximum benefit for
orthodontic services. This benefit may vary from as little as $500
to more than $2,000 depending on the type of insurance your employer
purchased. Charges for lost or broken appliances may not be covered
by insurance.
How Will Our Office Assist You with Your Insurance?
We will generate an insurance form on our computer that describes the
orthodontic problems, the required treatment, the length of treatment
time and the financial arrangements. Please bring a signed original
insurance form to our office so that we may have complete information.
We will submit this form to your insurance carrier for partial
reimbursement of your orthodontic fees.
How Can You Maximize Your Benefit?
Know what your benefits are before treatment begins. If you are not sure,
contact your insurance company or personnel office so that you know exactly
what benefit is provided. This information will tell you how much you can
expect to pay.
According to our contract with you, each monthly payment will be the
combined effort of both you and your insurance company. Once your
insurance limit has been met, monthly payments become your sole
responsibility. Although insurance may be a factor, please remember that
you ultimately responsible for all charges incurred in our office and that
you are personally responsible for any balance not paid by your insurance
company.
