SMILE Rewards You for Your Volunteer Hours!
SMILE (Saluting Models of Involvement, Leadership and Enthusiasm) recognizes patients of Drs. Chermak and Hanson who spend time volunteering in community service activities. The program reflects our involvement in helping to meet the needs of the communities we serve. Not only are we committed to providing quality orthodontic care for our patients, we also want to contribute to the Winston-Salem, Clemmons, and King communities.
SMILE lets you earn points for hours volunteered. You can redeem these points for prizes. You can also earn monthly and end-of-the-year SMILE big dollar awards.
You Can Earn $50, $75 or $100 Monthly
SMILE participants can earn Monthly Recognition Awards. Savings bonds of $100, $75, and $50 will be presented to the three people who earn the most SMILE points each month.
To be considered for the Monthly Recognition Awards, submit your completed SMILE Verification Form for the month by the 10th business day of the following month. Award recipients will be notified and will have their names posted in all three offices of Drs. Chermak and Hanson
Earning SMILE Points is Easy
- Volunteer at an approved organization. You will earn one SMILE point for each hour you volunteer.
- Keep a record of the hours you volunteer, using a Drs. Chermak and Hanson SMILE verification form.
- Have your supervisor sign and date your Verification Form. Include his/her telephone number in case we read to contact him/her. (If you do not have your form with you when you volunteer, you can have your supervisor sign and date a note showing the hours volunteered and then attach that note to your Verification Form.)
- Turn in your Verification Form for each month by the 10th business day the following month to qualify for monthly dollar awards.
- Keep a record of each month's Verification Form and use it when redeeming your points for awards.

The more hours you volunteer, the better your chances are for winning the monthly $50, $75, or $100 Savings Bonds awards and the annual Gift Certificates.
Qualify for SMILE Awards
Two Gift Certificates will be awarded annually to SMILE participants. One will go to the person with the most points for the year. The other will be awarded to the person whose name is drawn from all those who participated in the program during that calendar year.
One qualified hour will make you a contender in the drawing for the second Gift Certificate. For each additional 10 SMILE points earned, your name will be entered again in the drawing.
